Mission Hospice & Home Care is a charitable, non-profit organization that provides quality, compassionate end-of-life care and education for patients and families in the San Mateo and Santa Clara County area. Programs provide a Continuum of Care designed to serve patients – and their families – at every stage of a serious or life-limiting illness, from Community Education about advance care planning, to Grief Support services for those who are grieving a loss. The core of our work remains caring for people throughout the last years of life through Advanced Care, Transitional Care, and Hospice Care. Our Mission Houses – the only hospice houses in San Mateo County – provide a peaceful, home-like setting for patients at the end of life whose comfort cannot be managed in their own homes.
